Request Your Library Card and PIN

Eligible online students may use this form to request a UFV library card and PIN. Eligible online students are defined as those students who:

• are enrolled only in UFV Online courses; and
• are unable come to campus due to distance or physical disability.

Students who are able to come to campus are required to request their library card and PIN in person.

You do not need to have a UFV library card to take advantage of services for online students, but you do need to activate your library account, and you will need a PIN to check your library record. The easiest way to do this is to request a library card.
