Request for DVD or VHS Video

Please fill in a form for each DVD or VHS video you wish to borrow. Use the library catalogue to verify the call number.

Note that this form only applies to items that the UFV Library owns. To request an item not available from the UFV Library, use our interlibrary loan service.

Allow sufficient time for delivery (2-3 business days). Items are shipped via Purolator Courier to your home address (no post office boxes). A pre-paid return label is provided.

Please Note: This service is limited to current students and employees who do not come to campus for classes or work at least once a week and who are unable to come to campus because of distance or disability.
