Fill in this form to book a DVD or video owned by the UFV Library for a class show date. If you have multiple requests for titles throughout the semester, you may attach a list.
Please allow at least 3 business days to process your request.
If you are requesting a video that UFV does not own, please use interlibrary loans. Allow additional time for these requests.
